MODEL RELEASE AGREEMENT (ADULTS & MINORS)

I, [Model’s Full Name], hereby grant permission to [Creator’s Name] and Varsa Studios: Blank Space American Fork (hereinafter referred to as "Varsa Studios") to use my likeness in photographs and/or video recordings taken at Varsa Studios on [Date of Shoot].

I understand and agree that the content captured may be used for, but is not limited to:

✅ Marketing and promotional materials (social media, website, print, and digital advertisements).
✅ Commercial purposes, including but not limited to, advertising, studio branding, and content showcasing.
✅ Editorial, publication, and digital distribution, as needed by Varsa Studios.

I acknowledge that:

  1. I am voluntarily participating in this photoshoot/video session, and I waive any right to review or approve the final images or videos before their use.

  2. Varsa Studios owns an exclusive, perpetual, worldwide, royalty-free license to use, reproduce, edit, modify, publish, and distribute the content featuring my likeness.

  3. I will receive no compensation from Varsa Studios for the use of my likeness, and I waive any and all claims for remuneration related to the use of these images/videos.

  4. I release and hold harmless [Creator’s Name] and Varsa Studios, their representatives, contractors, employees, and any third parties acting under their authority from any claims, damages, or liabilities arising from the use of the images/videos.

  5. I understand that this release is irrevocable, meaning I cannot withdraw my consent after signing this form.

  6. If I am under 18 years of age, my legal parent/guardian must complete the Minor Model Release Section below.
